Social Worker - Ambulatory Care Network - Pediatrics - Weill Cornell
We are seeking a Social Worker for our Outpatient Pediatric clinic which includes both medically and socially complex cases. The Pediatric Social Worker will utilize clinical skills to forge meaningful relationships with patients and their families. Position involves a wide range of responsibilities including but not limited to biopsychosocial assessments, short-term counseling, risk assessment and treatment planning, crisis interventions, outreach/referrals to community providers, and other case management services.
